Screen form

The Screen form will support you to carry out sensory screening with each child and make a plan.

If the child has already completed their vision and eye health screen, they will bring their form with them.

If you haven’t already, download the Screen form and print a copy.

If you are unable to download the form, don’t worry. Questions on the form are shown as you go through this section of the module.

Check the following have been recorded:

  • Name of screener (if more than one screener, add both names)
  • Date of screen
  • Location where screening is taking place.

Information about the child

Check:

  • Information about the child section has been completed
  • Confirm consent from their parent/caregiver to take part in the screening.

Only continue if consent is given.

  • A child already wears hearing aids or
  • A parent/caregiver has concerns about their child’s hearing

Meet Basir

Basir is six years old and wears hearing aids. He is participating in a sensory screening programme at school.

Basir does not wear his hearing aids during the hearing screen and ear health screen. At the end of the screen Basir does not pass the hearing screen. Both his ears are healthy.

1. When notifying Basir’s parents, you should recommend that Basir is followed up by ear care personnel at the service he is already using.

Is this recommendation correct?


Yes is correct!

Basir already has hearing aids. He should be followed up by the service he is already using to check the hearing aids are working well.

2. If a parent/caregiver has concerns about their child’s hearing, should you complete the screen?


Yes is correct!

It is important to complete the screen.

If the child passes the screen, discuss the results with parent/caregiver. Plan a follow up screen within a month.

If the child does not pass the screen Refer to ear care personnel.
